Declaration and Terms and Conditions

1. DECLARATION AND AUTHORITY
1.1. I certify that I am the person filling this Form and IRD number and Photo ID Proof and bank account number given (if given) is my own.
1.2. I authorise TaxJack AMS Ltd to act as my tax agent, and obtain information for all tax types, and obtain information from Inland Revenue through all channels including electronic, for the current, future and previous 5 tax years.
1.3. I authorise TaxJack AMS Ltd to request my Personal Tax Summaries and/or file and sign my IR3 return(s) on my behalf, and have supplied all information necessary to carry out such service and I accept full responsibility for the accuracy and completeness of this information.
1.4. I hereby authorise TaxJack AMS Ltd to act on my behalf as my tax agent in all future years beyond this current year, to request my personal tax summary(s) or sign and file my return(s) at their processing fee which may change from time to time. This instruction remains in force until my further notice. This means in plain terms TaxJack AMS Ltd will recover my tax refund from Inland Revenue in all future years and be entitled to deduct their fee from my refund before paying me the balance.
1.5. I accept that I will be delinked from my existing tax agent or accountant.

2. COMMUNICATION
2.1. I agree that all correspondence from Inland Revenue relating to income tax will be sent to TaxJack AMS Ltd.
2.2. I understand that it is my responsibility to update my contact details to both TaxJack AMS Ltd and Inland Revenue if I change my address, phone number or bank account number.

3. FEE AND PAYMENTS
3.1. I agree to pay website TaxRefund application fee of 20% + GST of the refund amount, with a minimum fee of $25+GST.
3.2. I also agree to IR3 filing fee of $100+GST, if IR3 is required by Inland Revenue, and IR3 filing fee will be deducted from refund amount.
3.3. I understand that the Fee mentioned are for each financial year and are for all tax credits of all tax types.
3.4. I understand that once I have filled Tax refund Application with TaxJack AMS Ltd and they have assessed, requested or confirmed my PTS or filed my tax return, l am liable to pay Tax Refund Fee as mentioned above, even if I delink TaxJack AMS Ltd as my Tax Agent.
3.5. I authorise the Inland Revenue to credit my tax refunds to TaxJack AMS Ltd‘s specific Clients Funds Account and understand that my refund will be transferred after deducting the processing fees, to my bank account or a cheque can be posted to the address provided.

4. DISCLAIMER OF LIABILITY
4.1. I agree that TaxJack AMS Ltd have not performed an audit or review and therefore accept no responsibility for the accuracy of the information from which my return/PTS has been prepared. Further, the return/PTS has been prepared at my request and TaxJack AMS Ltd is not responsible on any grounds whatsoever, including liability for negligence, to any person.

5. TERMINATION OF CONTRACT
5.1. Either Party can terminate the contract at any time without notice. Any Fee due as per section 3 should be paid
